Dear New Client,
Looking forward to assisting you with achieving your goals for increased health and well being. In preparation for our work together there are two forms for you to fill out.

#2 Disclaimer and Informed Consent Form
The forms may be emailed, faxed to 512-358-0770, or brought with you to your appointment. Please fill out the forms in advance of your session, otherwise you will use some of your session time doing this.
